Grinspan’s syndrome
challenging diagnosis and treatment

We present a case of a 52-year old woman with severe and constant pain affecting the tongue and a burning sensation in the mouth. Intraoral examination showed diffuse, erythematous, extremely painful ulcerations especially in the tongue. The diagnostic hypothesis of erosive lichen planus was confirmed in the microscopic exam. The lesions were extremely refractory to the treatment, and the medical history of diabetes mellitus and hypertension support the Grinspan’s syndrome diagnosis. This rare entity is sparsely related, possibly due to the lack of information regarding this triad. This work also lists the reported cases in the literature and speculates if other conditions, as well as anxiety, depression and hepatitis C, play a role in the pathogenesis of this syndrome, enhancing the symptoms.
