Clinical crown lengthening and melanoplasty with high-power laser: a case report.

This study aimed to report a clinical case that combines clinical crown lengthening and melanoplasty using a high-power Nd:YAG laser, highlighting its applicability in restoring dentogingival harmony. The work is based on an updated literature review and a detailed clinical description, from initial diagnosis to postoperative follow-up. The patient presented altered passive eruption and gingival hyperpigmentation, initially undergoing periodontal preparation and subsequently the surgical procedures performed with a pulsed-mode Nd:YAG laser. The technique enabled precise incisions, immediate hemostasis, and minimal operative morbidity. Significant improvement was observed in dentogingival proportions, reduction of melanin pigmentation, and rapid healing without edema or bleeding, resulting in a more harmonious and satisfactory smile. The six-month follow-up demonstrated result stability and a high level of patient satisfaction. It is concluded that high-power laser therapy is an effective and predictable tool in aesthetic periodontal practice, offering significant advantages over conventional techniques and contributing to both the functional and aesthetic optimization of periodontal tissues.
